The forms below are necessary to complete your athlete's registration process. To complete the registration process online CLICK HERE. You must have a user account and be logged in. To get a User Account CLICK HERE.
To Immediately reserve a spot on a BHYFL team a completed Registration Form and the Registration Fee are required. The remaining forms will be collected by your Associations Team Mom. There is a maximum of 25 per team and a Maximum of 5 Older / Ligthers. If you wish for your participant to be an O/L, please email your request for an O/L status seperately.
To complete your childs Registration manually, simply click on the links below and you can either print them out and fill them out by hand or fill them out online then print them for mailing. All the forms below and payment in full is required to complete the registration process and reserve your childs spot on a BHYFL Team.

Regargless of the registration process you utilize, a current medical release form is required for your athlete and it is to be completed by a Licensed Health Care Professional that is authorized by the State of South Dakota to complete the form. The BHYFL offers a "Physical Day" in July (see events schedule) at Varick Family Chiropractic for only $15.
